| Against | Removing the requirements to allow nurse practitioners to practice independently at first seems like a good idea because they are cheap labor relative to a physician. However, you get what you pay for. NPs have less than half the training of an MD. They are great at treating everyday illnesses, but they WILL miss many serious malignancies which often have subtle initial clinical presentations. The doctor shortage in Nevada is driven by low reimbursements and student loan repayment programs that far lag behind the rate of medical training cost inflation. In other words, the state and insurance companies are paying less and is now shocked that the supply of physicans is falling in Nevada (while rising rapidly in other parts of the country). The psych/social/medical needs and strengths of each individual family determines the level and intensity of collaboration. My son takes me to my doctor's visit every other month. We always request to see the physician and not a nurse because the doctor knows the full picture and can tell my son exactly what is going on with my health. Nurses have their place in the doctor's office, but I would never be comfortable going to see a practice run solely by a nurse and my kids would never be comfortable with it either. Nurses (no matter how much training they have) cannot provide the same level of care that a physician can. Period. If nurses want to take on the patient responsibility, financial burdens, and education required by physicians than they should go back to medical school. Trying to pass an underhanded law to essentially give nurses the right to practice medicine without the supervision of a physician is bad for all involved--especially the patients. If you thought the Hep-C outbreak in Vegas was bad, wait until nurses are practicing sans docs. I with Type I diabetes for 38 years and my husband with Type 2 for about 3 years. We both work in Healthcare and were relocated to Nevada nearly 7 years ago from So. California. I, being a long term patient of diabetes, realized almost immediately that there were so few physicians here that specialize in diabetes care. The PCMH model improves the quality of care because it capitalizes on the unique expertise of each member of the patient's health care team. Nurse Practitioners are a vital part of the health care team, but they cannot fulfill the need for a fully trained physician. Nurse practitioners are trained to follow through on the treatment of a patient after a diagnosis and to implement protocols for chronic disease management. | Against | Physicians have nationally regulated educational requirements and have to go through highly regulated and heavily supervised training to practice independently. Nurse practitioners have significantly less education and that education is not standardized across the country. They also have to go through dramatically fewer clinical training hours in order to practice. Additionally, the bulk of nurse practitioner training is done online (how one learns to care for another online is still beyond my comprehension). It is clear that to let nurse practitioners practice without physician supervision is extraordinarily irresponsible. There is also a notion out there that this bill will improve access to care. This is not true. There are countless nurse practitioners practicing throughout the state in appropriate physician-supervised roles already. | Against | I am a Family Medicine doctor with an MD degree. I have experience with many different health care providers, including nurse practioners. There is a role for nurse practioners, but that role requires supervision. The training a nurse practioner receives does not compare to the education an MD or DO obtains. When caring for complicated patients, this difference becomes clear. Unfortunately, there are times that nurse practioners are caring for patients who are too complicated and the nurse practioner is not aware that the patient is beyond their scope of practice. One of my patients is an example of this. This patient had seen a nurse practioner for over a year before seeing me. She was told that she had diabetes even though her labs for diabetes were normal. What was missed was a very abnormal white blood cell count that was never addressed. By the time I cared for this patient, her white blood cell count increase to 5 times the normal limit. My patient had untreated leukemia for 1 year. Treatment should have been started a year prior when she had her first abnormal value. To practice independently under the governing bodies of the State Board of Nursing and the Pharmacy Board and without burdening physicians and their liability concerns, this would enable more people to have access to a healthcare provider. I am confident that an Advanced practice nurse (APN) provides quality healthcare at more affordable prices. When needed, an APN collaborates with other healthcare providers and makes appropriate referrals to higher level of care. I am writing you in support of Senate Bill 69 and the legislative efforts to remove the collaboration restrictions on Nurse Practitioners in Nevada. As a patient I believe I am a partner in my health care, I want to let you know that I support allowing Nurse Practitioners to work to the full scope of their education and training. I choose to see a Nurse Practitioner and my Nurse Practitioner has provided me with high quality, patient-centered care. I recognize that Healthcare in Nevada is going through significant changes, allowing Nurse Practitioners to work to the full extent of their abilities offers a pathway towards improving healthcare in Nevada. It also means access to healthcare for myself, my family, friends and other Nevadans. As your constituent, I encourage you to support SB69 and the legislative changes to increase access to healthcare for all Nevadan’s. Eliminating the physician oversight restriction of Advanced Practice Registered Nurses will be a significant step towards that goal. I have been trying to VOLUNTEER my services as a Nurse Practitioner at a free teen clinic here in Las Vegas for over 3 months, but have been unable to do so because I can not find a collaborating physician who is willing to volunteer their time for this important cause. If the collaborating physician requirement were removed, I could be providing much needed medical care to another underserved population and positively impacting the life's of members of the community. | Feb-22-2013 |
